Do you fall asleep during the day when you don’t want to?
Do you take a nap during the day and feel worse after?
Do you routinely fall asleep after midnight?
Do you routinely fall asleep after 3 am?
Do you work nights or swing shift?
Are you only able to sleep during the day?
Do you have nightmares more than once a month?
Do you have frequent headaches?
Do you think you have depression or anxiety?
Do you take sleeping medications, even non-prescription?

Does your child fall asleep during the day at school?

Does your child routinely fall asleep after 3 am?
Does your child have night terrors?
Do you think your child has ADD or ADHD?
